Registration Packet: New clients are asked to complete a registration packet at their first appointment. Please download the registration packet from the links below (based on client age) and complete at your convenience before the first appointment.
- New Client Minor Registration Packet (ages 17 and younger)
- New Client Adult Registration Packet (ages 18 and older)
The registration packet includes 4 documents:
- New Client Registration Form – complete and bring to your first appointment
- Service Agreement – review and keep for your records
- Notice of Privacy Practices (HIPAA)– review and keep for your records
- Consent and Signatures Form – sign and bring to your first appointment
